Executive Board/Committee Nomination Form
Are you collecting nominations for next year’s open board positions? Consider using a Data Only Sales Form to collect self-nominations for open board positions. Using a Form Condition, you can restrict access to the voting form to only allow those who should be voting to vote.
We have a sample form available that we can add to your account to help you get started. Just submit a ticket with your request through our help desk.
- You can edit, delete, and add data fields on the Product & Data Fields tab.
- We also suggest entering a CC'd email address on the Email tab so someone is notified when the form is submitted.
- We have added a new feature that will tabulate the votes for you! You will find this feature under Store & Forms > Forms. Click the Summary Button to the right of your form.
